Hospital AIs and insurer AIs are now fighting over the bill

The Story
Hospitals use ambient scribes and coding AIs to find extra diagnoses; insurers use AIs to deny them. Blue Cross analysis of tens of millions of claims says AI-enabled coding intensity added on the order of $0.9–2.3B. Maternity “acute posthemorrhagic anemia” jumped ~4% to 12% at high-growth hospitals while transfusion rates stayed flat — more codes, not more blood.
Why It Matters
Two institutions hired ghosts to argue with each other, and the patient is the battlefield. Money + identity + paperwork as the real AI war.
